AIMA forms and templates, what each one is for

In short

  • AIMA publishes two kinds of documents under Impressos e Minutas, forms (Modelo 1 to 7 and the Equality Statute ones) and declaration templates (minutas) that the applicant or a third party completes.
  • Modelo 1 is for applying for or renewing a residence permit and also covers family reunification, long-term resident status, the Residence Permit for Investment and the EU Blue Card.
  • Modelo 4 is the Termo de Responsabilidade (statement of responsibility), and the signature must be certified by a notary, lawyer or solicitor unless it is made before an AIMA representative.
  • The Modelo Manifestação de Interesse belongs to a regime that ended in June 2024 and cannot be used to apply for residence today.

AIMA’s Impressos e Minutas page gathers dozens of PDFs with names that mean little to someone seeing them for the first time. This guide groups them by the situation in which they are used and says, in plain words, what each one is for. The content of the forms is not reproduced here: always download the current version from the official page, because AIMA updates the files.

Two distinctions help. The forms (impressos: Modelo 1 to 7 and the Equality Statute ones) are applications: they ask AIMA to do something. The templates (minutas) are standard declarations that the applicant, or a third party (employer, school, the other parent), completes to prove a fact. The site also has a templates page, but those are message texts for writing to AIMA or other bodies, not official forms.

To apply for or renew a residence permit

  • Modelo 1. The main application form. It has boxes to tick for what you are asking: grant or renewal of a temporary or permanent residence permit, of long-term resident status, of the Residence Permit for Investment (and the permanent ARI) and of the EU Blue Card, or else family reunification. For reunification it asks which Portuguese diplomatic mission the family will use if they need a visa. The same form covers change of data and duplicate of the document (poor condition, loss or theft, new address, new marital status), and it carries a checklist of the documents handed in: passport, health insurance or SNS registration, means of subsistence, accommodation, IRS and proof of basic Portuguese. The guide to AIMA and the residence permit shows the route it fits into, and the one on renewing covers renewal.
  • Modelo 2. Request to extend the stay under a temporary stay visa or a residence visa, with the grounds the law sets for each case. One of them is having a residence permit or residence visa application pending, which matters if your residence visa expires before your AIMA appointment.
  • Modelo 3. Extension of the stay, validity or duration of a Schengen visa.
  • Modelo 7. Request for a certificate from AIMA.
  • Declaration of accommodation template. The sworn declaration of your address, which the AIMA guidance lists as part of the proof of housing.
  • Request for an extension of a deadline. Used to ask for an extension of a deadline in your case.
  • Request to withdraw an application. For someone who decides to withdraw an application already made.
  • Declaration of stay in national territory. A sworn declaration by the applicant that they stayed in Portugal between two dates without leaving. AIMA will tell you whether your case needs it.
  • Declaration of compliance with the ARI requirements. For people applying for the Residence Permit for Investment.

Responsibility and Modelo 4

Modelo 4 is the Termo de Responsabilidade (statement of responsibility). Whoever signs it declares themselves responsible for a foreign national as regards the conditions of stay in Portugal, removal costs if necessary, and compliance with Portuguese law; the form refers to article 12 of Law 23/2007. AIMA says the signature must be certified by a notary, lawyer or solicitor, unless it is made before an AIMA representative. In family reunification it is one of the resident’s documents, as the family reunification guide explains. Read the text carefully before signing, because removal costs can fall on you.

EU citizens and their family members

Modelo 5 is the application for the residence card of EU, EEA and Swiss nationals and their family members: first application, renewal, change of data and duplicate. The guide to EU citizen family members describes that regime.

Equality Statute

There are three applications for Brazilians resident in Portugal, depending on what is requested: rights and duties, political rights and the combined rights and duties and political rights form. The Equality Statute guide explains who can apply for each.

The form that no longer works

The Modelo Manifestação de Interesse (expression of interest form) is still listed on the AIMA page, but it belongs to a regime that ended. Decree-Law 37-A/2024 of 3 June repealed the residence procedures based on expressions of interest, in force from the day after publication. Do not use that form for a new application. For the routes that exist today, see the visas guide. The decree-law safeguards procedures already started before it came into force, so if yours began earlier, ask AIMA how it is treated.

How to use this list

  1. Identify your situation and the channel you will apply through (counter, portal or email), because what you hand in varies.
  2. Download the PDF directly from AIMA and read the instructions before filling it in.
  3. If you are unsure which one your case needs, call AIMA on (+351) 217 115 000, Monday to Friday, 8:00 to 20:00.

Frequently asked questions

Which form do I use to apply for a residence permit?

Modelo 1, which AIMA lists for applications and renewals of temporary or permanent residence permits, long-term resident status, the residence permit for investment, the EU Blue Card and family reunification. Check at the counter or in your case page whether there is an electronic channel for your case.

What is Modelo 4 and how is it signed?

It is the Termo de Responsabilidade. AIMA says the signature must be certified by a notary, lawyer or solicitor, unless it is made before an AIMA representative.

Can I use the Modelo Manifestação de Interesse?

Not for a new application. The residence procedures based on expressions of interest were repealed by Decree-Law 37-A/2024 of 3 June. If your case began before that date, ask AIMA which regime applies.

What is the difference between AIMA's forms and the site's templates?

AIMA's forms and templates are official documents that go into your case. The templates on this site are message texts, for example to ask about the status of a case, and do not replace any official form.
